and why is it not generally visible?

By default, a moderator's edit is never shown on phpBB3, unless they supply a statement in the "reason to be edited" line. Of course, the side effect of that is, even if they edit their own posts, it always shows up in the log. I think that might have changed in a recent phpBB version, though, cause I don't spam the moderator logs nearly as bad as I used to.
